Diabetes Community Outreach Worker/Promotor(a)

Position Description

POSITION TITLE: Part-Time, Diabetes Community Outreach Worker/Promotor(a)

(10-15 hours/week)

POSITION SUMMARY:

This position will provide support to the strategies of the Riverside Community Diabetes Collaborative (RCDC) by providing bilingual, bicultural outreach and education in the community in the area of diabetes. The Diabetes Community Outreach Worker/Promotor (a) will connect community residents to the programs of RCDC and other resources and referrals for the prevention and management of diabetes.

ESSENTIAL POSITION DUTIES AND RESPONSIBILITIES

1. Receive training on diabetes education, including the American Diabetes Association’s Por Tu Familia curriculum, leadership development, public speaking, team building, referrals, and resources.

2. Assist community residents in accessing services and presenting direct messages about diabetes prevention and healthy lifestyle choices.

3. Attend meetings in the community relevant to RCDC and its activities.

4. Attend Conversation Map classes and assist facilitator, also providing follow up with participants of Conversation Map classes through phone calls and face to face encounters.

5. Refer to other community resources both in the local community and on line.

6. Conduct and assist with community educational workshops.

7. Provide documentation and maintain tracking records for attendance and evaluation components.

8. Assist with the participation in community health fairs and events.

9. Must have flexible hours to work evenings and weekends.

10. Must have transportation to travel to presentation sites, community meetings, and outreach opportunities.

Minimum Qualifications:

Education:

• High School Diploma desired.

 Experience:

• Participation and/or leadership role in community groups

• Be a resident of the greater Riverside area

 Knowledge and Skills:

• Ability to receive and follow direction

• Ability to effectively interact with a diverse community population

• Possession of good verbal and written communication skills in both English and Spanish

• Ability to work independently with minimal supervision; self-motivated

• Ability to present relevant material to individuals and large groups

• Computer literate

• Proficiency in operations of various office machines (Phone, Fax, Xerox) and computer programs (Microsoft Word, Excel, etc)

• Must have a valid California Drivers License and insurance

 Abilities:

• Ability to set goals and to work toward meeting those goals in an organized manner

• Ability to organize work and to work on multiple projects at once

• Ability to maintain the highest level of confidentiality in all work assigned

• Ability to communicate (read, write, and speak) in English and Spanish

• Ability to present a professional appearance both in dress and demeanor

• Ability to sit for long periods of time

• Ability to follow directions
